[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide mental health and substance use disorder navigation services to the government authority located in Colorado.
- Screen clients for mental health and substance use service and support needs and the barriers to accessing needed services and supports;
- Connect clients to the services and supports they need;
- Provide support to ensure that the clients are engaged in services;
- Complete follow-up with the clients to identify access barriers and facilitate a modified service plan;
- Monitor the individual and community-level needs and barriers and collaborate with local partners to address the needs.
- Navigation is a non-clinical, time-limited intervention and is driven by client needs rather than diagnosis and treatment, it involves care coordination with service providers and agencies to facilitate improved and expedited connections to mental health and substance use care.
(2) All questions must be submitted no later than April 17, 2019.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
