[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide wellness platform to following
- Health Risk Assessment – platform offers a comprehensive health risk assessment and can integrate with health risk assessment of health carrier Kaiser Permanente
- Customizable Challenges – platform offers a wide variety of holistic well-being challenges in a group and team, individual and peer to peer format. Challenges must be customizable and focus on all areas of well-being including physical, emotional and mental, community, social, career and financial well-being.
- Biometric and Cancer Screening Data Integration – platform is able to securely integrate with our health insurance carrier Kaiser Permanente to report completion of required biometric and cancer screenings.
- Health Education Resources – platform will offer several modalities for health education and lifestyle improvement classes including on-demand webinars, health education modules, recipe and exercise databases, health trackers, daily quizzes, etc.
- Wireless Tracking Device Integration – platform will integrate with a wide variety of wireless tracking devices brands via aggregator and allow synching of physical activity, nutrition, sleep and other health data as requested.
- Incentive Management – platform will track and accumulate points to determine successful program completion and have the capability to distribute incentives directly to qualifying participants via online rewards mall, gift cards, etc.
- Reporting – platform will have the capability to produce on-demand program reporting, data analytics, population trends, etc.
- Mobile responsive and App – platform will be mobile responsive or will have an App available for download to access the portal remotely from mobile devices.
- Health Coaching Services – platform has the capability to provide in bound and out bound, telephonic, email, Skype and on-site health coaching services for wellness program participants as requested.
- Communication Materials and Marketing – Develop requested communication materials pertaining to our wellness program and challenges; assist in establishing timelines, drafting, revising, and finalizing marketing materials and flyers to distribute to workforce.
- Innovation – provide and recommend innovative ideas and user-friendly products, programs and services to ensure a competitive, valued and cost-effective wellness program
(2) A pre-proposal conference will be held on November 28, 2018.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
