(1) Vendor needs to provide, develop and implement a statewide plan to care coordination for high-risk individuals including comprehensive behavioral health care system, policy development, service provision and coordination, program monitoring and evaluation, and administrative oversight of the public behavioral health system to the government authority located in Denver, CO.
- The Behavioral Health Transition Specialist Program is designed to minimize the risk of readmissions to hospitals, withdrawal management, arrest and to ensure an individual's stability in the community.
(2) All the questions must be submitted no later than November 2, 2018.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
