[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide biometric screening and incentive management reporting services for venipuncture blood draw, lipid and glucose blood draw, cholesterol, triglycerides, glucose and cholesterol ratio, blood pressure, pulse, height, weight and body mass index calculation, utilization of a walk-in facility, lab will notify participant immediately when any levels reach critical value, professional medical examiners performing the screenings, re-testing is needed employee will be responsible for any costs incurred and for arranging re-testing with their health care provider, employees enrolled in a medical plan the authority government located in Fort Collins, CO.
(2) All questions must be submitted no later than October 11, 2018.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Onsite and Offsite. Vendor needs to carry work in their office location.
