[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide case management services to conduct intake appointments with new applicants to determine their needs and design a case plan to work toward meeting these unmet needs, assist the probation officer and problem-solving court coordinator to develop and implement an onboarding process for new clients, assist clients to apply social assistance programs as appropriate for each client, including but not limited to SNAP benefits, housing vouchers and other housing assistance, SSI and SSDI, Medicaid and Medicare, Medicaid waivers, TANF, resources for individuals with developmental disabilities, and WIC, help clients maintain these resources once secured and assist clients to schedule mental and physical health appointments and attend these appointments with clients as necessary to the government authority located in Denver, CO.
(2) Contract period will be one year.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
